Annual Community Health Worker Conference

Since 2016, we have held an annual conference and we invite and encourage anyone who is interested to attend. Our conferences are full of helpful information in regards to your health and the impact you can have on your community. We also offer continuing education credits at our conference for Certified Community Health Workers.

Vendors

Each vendor will receive conference registration, meals, and snacks for (1) person for the September 19th and 20th event. If you have additional staff that would like to attend the conference from your organization, please supply their name and payment for their registration. Please check the appropriate box. Approval is subject to space availability. Platinum, Gold, Silver, and Bronze sponsors will receive priority vendor booth locations. Please check in with event organizer on the day of the event to get vendor booth assignment. Payment is due 30 days prior to scheduled event.

Conference Awards

Each year before the KYACHW conference, KYACHW members have the opportunity to nominate a CHW for the following awards.

This award is to honor a current CHW, former CHW, or CHW supervisor who has gone above and beyond throughout their career to contribute to the CHW profession. This awardee should be an individual who demonstrates dedication towards achieving equity and removing barriers related to social determinants of health. This individual is devoted to unifying, promoting, and sustaining the CHW profession across the Commonwealth of Kentucky.

Outstanding CHW Award

This award is to honor an individual CHW who has made outstanding contributions within the CHW profession. This awardee should be an individual who demonstrates leadership among other CHWs and within their community. The Outstanding CHW is an agent of change who boldly advocates for the profession and their community, and exemplifies what it means to be a Community Health Worker.

To honor an individual who has supported and advocated for the community health worker profession in Kentucky.  Awardees can be anyone who champions the CHW profession.
