Join the Kentucky Association of Community Health Workers (KYACHW) and become part of a statewide network of Community Health Workers (CHWs), patient navigators, and supporters working together to advance community health in Kentucky. Membership is annually from January – December.  Each individual new or returning must fill out a new application each year.

  • Community Health Workers
  • Patient Navigators
  • Community Members
  • Anyone involved in or supporting community health work.

Membership Types and Dues

Membership is based on calendar year (January- December) Membership numbers will begin with the current year and updated as payments are processed. Non-payment of timely dues will result in a reactivation fee of $10 plus membership per individual. 

Individual Membership, please pay upon application, invoice option is used for organizations.

  • Create an account on the Association Sphere KYACHW membership portal.
  • Choose your membership type and complete the registration
  • If joining under and organizational membership, enter the unique coupon code provided by your organization at checkout to receive the discount.
  • If you need to add more members beyond your purchased slots contact Sam Bowman KYACHW Membership Chair for additional codes.

Organizational Membership Instructi ons

If your organization purchases Tier 1 or Tier 2 Membership:


• The team lead should start the process first.
• The team lead will complete the group registration and select a  payment option or submit payment for the group.
• Once payment has been received or other payment options have been selected , the team lead will receive a coupon code via email.
• The team lead can then share that coupon code with the remaining team members. 
• Each additional member will complete their own application on the KYACHW website and use the coupon code so they  do not have to pay to finalize membership.
