All mentorship must be documented and submitted using the KOCHW Mentorship Tracking Form

Legislative Required Courses only have to be completed once and not repeated each year.

• $50 Recertification Fee

• State certification for Community Health Workers
• Certification # will begin with KCC
• Required for official recognition as Kentucky Certified CHW
• Used for continuing education and recertification
• Fees are paid when applying or recertifying
• Official state process through Kentucky Office of Community Health Workers.

• A professional membership organization for CHW’s across KY
• Focuses on advocacy, networking, education, and support
• Offers conferences, trainings, leadership opportunities, and connections with CHW’s statewide
• Membership dues are separate from certification
• Renewed annually in January

KOCHW certifies you and KYACHW offers networking to support your CHW journey. Both organizations work closely together, but they are not the same. many CHW’s choose to be part of both. KYACHW membership is open to everyone- certification is not required. We welcome certified CHW’s, students, future chw’s and supporters alike.
